Vielen Menschen am Lebensende ist es häufig aufgrund ihrer physischen oder kognitiven Einschränkungen nicht mehr möglich sich in der gewohnten Form, insbesondere mittels Lautsprache, zu kommunizieren. Im Rahmen dieser Arbeit wird daher der Frage nachgegangen, welche Möglichkeiten die unterstützte Kommunikation für Menschen mit Kommunikationseinschränkungen am Lebensende im Hospiz- und Palliativbereich bietet. Es werden die Grundzüge der unterstützten Kommunikation und deren Möglichkeiten, Anwendungsgebiete und Einsatzbereiche aufgezeigt. Der Fokus liegt dabei insbesondere auf den Bedürfnissen von Menschen am Lebensende und wie, unter Anwendung unterstützter Kommunikation, diese befriedigt werden können. Die Ergebnisse der Literaturrecherche zeigen, dass die unterstützte Kommunikation für den Hospiz- und Palliativbereich viele Möglichkeiten im Bereich der körpereigenen Hilfsmittel ebenso wie der körperfremden Kommunikationsmittel, welche in nicht-elektronische und elektronische Hilfsmittel unterteilt werden, bietet. Es wird auf die Möglichkeiten niederschwelliger Kommunikationsmittel, natürlicher und komplexer Sprachausgabegeräte, dazu passende Voice- und Message Banking sowie Ansteuerungsmöglichkeiten mittels Mundmaus oder Joystick eingegangen.

All diese Hilfsmittel können je nach individuellen Einschränkungen, Einbezug von Betroffenen Angehörigen und dem Pflegepersonal zu Hause, sowie in Pflegeanstalten angewendet werden und so zu einer verbesserten Lebensqualität für sterbende Menschen beitragen.

At the end of their life, many people face restrictions of using their voice and also other forms of communication, often due to physical or cognitive limitations. This study examines and analyses the opportunities of augmentative and alternative communications (AAC) people with communication impairments at the end of their life, in the hospice or more general in palliative care sector. Basic concepts and technologies of AAC, areas of application, are shown. The focus is directed towards typical social and human needs of people at the end of life and how these needs can be addressed in a satisfying way for all stakeholders. Results of a literature research show that supported communication for the hospice and palliative care sector brings benefits through endogenous aids as well as external communication aids (non-electronic and electronic aids). The possibilities of low simple communication methods, natural and complex speech output devices, voice-banking and message-banking as well as control options using a mouth mouse or joystick are discussed. All of these tools can be used at home and in nursing homes, depending on the individual limitations, the involvement of those affected by relatives and the nursing staff, and thus contribute to an improved quality of life.
