It has been shown that a significant increase in demographic change in older people can be expected. Besides, it has a massive influence on how they cope with their everyday life, which is indicated in the multifactorial phenomenon of frailty. In addition to physical limitations, frailty also triggers the loss of autonomy and self-determination. To get an interdisciplinary cooperation, it is essential, therefore to take needed countermeasures. The knowledge of the needs of older people, the phenomenon of frailty and the framework conditions are immensely important. The aim of this work is to present the phenomenon of frailty as well as to show the influence of interdisciplinary cooperation in the geriatric setting on the phenomenon of frailty.
To achieve this goal, a literature search was carried out.
The research showed that staying in their own four walls meaning staying at their home. is an essential aspect for older people. The timely recognition of a frailty risk and the resulting therapies can curb the phenomenon of frailty. An individually adapted treatment strategy requires cooperation and coordination between different disciplines.
Nevertheless, there are many disruptive factors in the cooperation. The main problem is involving the willingness of all people to cooperate. Communication skills and self-reflection form the basis of cooperation. However digital media is needed to enable data access at all times. The positive result of which is that the people affected benefit from interdisciplinary cooperation, as well as all the health care system involved.
The empowerment is reflected in the common approach and the active cooperation of older people.
